We’re collaboratively designing population health management strategies to advance provider capabilities and address disparities.

The Population Health Management Initiative is a California collaboration of the Department of Health Care Services, Kaiser Permanente and Community Health Centers (CHCs). We’re supporting a cohort of 32 CHCs, which serve 1.1 million people, to improve their population health management capabilities and align with Medi-Cal policy and payment reform initiatives.

Guides and tools to support population health management best practices

We’ll be continuously offering resources including guides, tools, and best practices around expanding specific population health management capabilities. These resources support anyone interested in population health improvement for providers and practices in the Medi-Cal delivery system.

Our program is collaboratively designed and tailored to the needs of California Community Health Centers

We are working with Community Health Centers across the state to collaboratively design population health management strategies that work for them and the patients they serve. We are aiming to advance Community Health Center capabilities in population health management that address people, process, and technology.

Our goals include:

Improve quality of care and equitable health outcomes of people served by CHCs​

Enhance access through team-based care and care redesign at CHCs​

Improve CHC patient engagement and experience of care

Improve CHC technology enablement to optimize population health capabilities
