The PHM Initiative is working in partnership with California Department of Health Services to advance health equity by improving care quality and efficiency. This means increasing access to care, improving health outcomes, and reducing health disparities. This requires a deep investment in systems change from all of the healthcare partners.
Guides and tools to support population health management best practices
We’ll be continuously offering resources including guides, tools, and best practices around expanding specific population health management capabilities. These resources support anyone interested in population health improvement for providers and practices in the Medi-Cal delivery system.
Our program is co-designed and tailored to the needs of California Community Health Centers
We are working with Community Health Centers across the state to co-design population health management strategies that work for them and the patients they serve. We are aiming to advance Community Health Center capabilities in population health management that address people, process, and technology.
Our goals include:
Improve quality metrics and address disparities in specific regions
Focus on child health, maternal health, and behavioral health
Improve patient engagement and experience of care
Increase access to primary care services through team-based care
