The current pressures on our health care systems are undeniable and the vulnerable populations we serve face tremendous uncertainty. This time brings to mind the birth of the Community Health Center (CHC) movement, which emerged during the civil rights era, born from struggle and an unwavering commitment to serve those most in need of care. It was made possible by dedicated community organizing and tremendous innovation and has remained a resilient model for half a century.
Now, we are called on to meet this moment with the same innovation and determination, standing together in steadfast dedication to serve our communities in total health. At the same time, we are hearing from so many providers about the mental and emotional weight they carry. It reminds us that sustaining quality care requires more than individual effort; it also requires caring for ourselves and for one another.
What does that look like in practical terms? It can mean a mindful and personal focus on rest, renewal, and connecting with the underlying reasons we do this work. It can also mean supporting each other by checking in or lending a hand when a colleague is having a hard day.
At the PHMI Program Office, along with our Practice Transformation Partners, we know that advancing population health management capabilities comes down to the tireless work of the CHC workforce, who’ve dedicated their lives to this mission. Our commitment remains to both improved patient health outcomes, and the wellbeing of those who provide it.
History shows us that periods of great challenge often birth the most meaningful progress. By supporting one another and maintaining our commitment to those we serve, we honor the legacy of our movement while building resilience for the future.
