In March, PHMI stakeholders, including representatives from our cohort of 32 CHCs, Regional Associations of California, DHCS, CPCA, and Practice Transformation Partners came together for an immersive Statewide Learning Session aimed at supporting CHCs develop a robust set of practical ideas and next steps towards achieving improved population health outcomes.

We know that nutrition is critical to health and well-being. And when people are hungry or cannot access nutritious food, they’re less likely to get or stay healthy. Many of these people are served by our PHMI cohort of CHCs.

Beautiful, rural Mendocino County is home to a diverse population of families, service industry workers, loggers, fishermen, and a growing Hispanic community. The area also faces significant challenges with substance abuse, high rates of fentanyl-related deaths, and homelessness.

As we move forward into 2025, we’ve been looking back upon 2024 and want to express how much gratitude we have for you, your commitment, and your participation in PHMI. You are all critical to the success of this initiative.
