The PHM Initiative Cohort
Our selection process was designed to improve patient lives in the present, while creating a model for the future
Kaiser Permanente, Department of Health Care Services, and California Primary Care Association worked together to identify a geography-based cohort of Community Health Centers to participate in the PHM Initiative based on the following considerations:
Population size and geographic characteristics (rural, urban)
Interest in value-based payment
Our first cohort of Community Health Centers represents over 300 sites spanning 8 counties.
