Quarterly Implementation Plans help each health center organize and track progress toward its PHMI goals. Use the links below to access your health center's QIP shared folder, where you can view and update your current implementation plan.

Access: Each shared folder / file is available only to the authorized members of your health center's team and designated PHMI staff. If you are unable to access your QIP or have questions, please contact phmi.ta@jsi.org or reach out to your Coach.
