The Adults With Preventive Care Needs Change Package helps health centers define their 2026 population health management vision and develop high-impact SMARTIE goals for breast, cervical, and colorectal cancer screening. It includes a SMARTIE goal guide, key driver diagrams with evidence- and best practice-based clinical, access, patient engagement, HIT, and equity strategies, equity considerations, and examples of outcome, process, and balancing measures. The package supports health centers in aligning evidence with local context to reduce screening disparities, strengthen outreach and navigation, improve access and follow-up, and build a PHMI measurement strategy.
