These seven modules cover a range of empanelment fundamentals, including the benefits of investing in a well-functioning empanelment environment as well as role-specific skills and considerations for support staff, panel managers, PCPs, and clinic leaders.
Each module has been designed for specific target audiences. The modules can be taken individually as stand-alone learning opportunities, or together for more comprehensive learning of key empanelment concepts and skills:
- Module 1. What is Empanelment? This introductory module defines empanelment and describes its benefits, goals and key concepts. The PHMI eLearning courses and modules have been developed to supplement PHMI health centers' work with coaches and support in implementing key activities outlined in the PHMI Building the Foundation and Population of Focus Implementation Guides.
- Module 2. The Value of Empanelment This module describes the value of empanelment and explains how to calculate the return on investment of a well-functioning empanelment environment.
- Module 3. The Benefits of Empanelment for Providers and Patients This module includes video clips of providers talking about how empanelment impacts them and their patients and provides responses to common provider concerns about empanelment.
- Module 4. The Role of a Panel Manager This module explains why a panel manager is critical to empanelment success, describes the responsibilities of this role, and defines steps that help ensure a panel manager’s effectiveness.
- Module 5. Empanelment Policies and Procedures This module identifies the core components of empanelment policies and explains the importance of having policies and procedures in place. It also provides a sample policy template.
- Module 6. Talking with Patients About the Importance of Continuity This module uses interactive scenarios to demonstrate how to talk with patients about the benefits of continuity during scheduling calls, and provides a downloadable file of scripts that can be used for training.
- Module 7. Skills Lab: Calculating Provider and Patient Perspective Continuity This module explains how to manually calculate continuity from the patient perspective and the provider perspective.
