This session during the PHMI June Learning Sesssion supported health center teams in identifying common PDSA challenges and designing a small, practical test of change to begin applying immediately in their population health management work. Participants used the Instant PDSA worksheet to design their next small test of change—to test it right away! Each team practiced testing with a single patient, single staff person, or single part of their process.
