Regional Learning Sessions are one day in-person convenings designed to build a community of peers and support CHCs as they share identified promising practices and emerging challenges for population health management.

Learning Objectives

By the end of this convening, participants:

  1. Built a stronger sense of community amongst peers working on PHMI regionally.
  2. Applied strategies that further advance CHC capabilities related to Population Health Management (PHM).
  3. Discussed current and emerging PHM challenges & share promising practices with their peers.
  4. Described actions that support a foundation of equity & trauma informed culture to address disparities in care.
  5. Identified shared goals and available support from PHM stakeholders to strengthen collaboration.
