Partnering with Patients as Equals in Co-Designing Primary Care: Examples and Tools from the LINCC Project

This resource was developed out of a 2014-2015 Initiative from Kaiser Permanente Washington Health Research Institute. The goal was to design and implement a new primary care clinic role, called a lay staff person, who would connect patients with community resources. This led to the creation of Community Resource Specialist. This guide helps clinics learn how to implement an approach that engages patients in care design and quality improvements.
