The PHMI Clinical Guidelines: Adults Living With Chronic Conditions compile evidence-based recommendations and PHMI measure guidance for Controlling High Blood Pressure and Glycemic Status Assessment for Patients With Diabetes. Drawing from the American Heart Association (AHA), American College of Cardiology (ACC), American Diabetes Association (ADA), U.S. Preventive Services Task Force (USPSTF), and Kaiser Permanente National Guideline Program, this resource summarizes current recommendations for hypertension and diabetes care.

The guidelines address blood pressure screening, diagnostic confirmation using office, ambulatory, and home blood pressure monitoring, treatment thresholds, antihypertensive therapy, lifestyle modification, and follow-up.

They also summarize diabetes screening and diagnosis, HbA1c targets, blood glucose monitoring and continuous glucose monitoring (CGM), pharmacologic management, including insulin and GLP-1 receptor agonists, cardiovascular and kidney risk reduction, and management considerations for metabolic dysfunction-associated steatotic liver disease (MASLD).
