PHMI Social Health Evidence: Children

This resource summarizes evidence on how pediatric social health interventions may affect PHMI priority measures, including childhood immunization status, adolescent immunization, well-child visits during the first 30 months of life, and child and adolescent well-care visits. It reviews interventions designed to address social risks and barriers to care, including multi-need social needs screening and navigation, medical-legal partnerships, transportation assistance, appointment reminders, translation services, financial coaching, food vouchers, and other clinic access strategies. 

It highlights evidence of improved well-child visit attendance and routine immunization associated with several approaches, including successful resource connections, transportation and language support, financial coaching, and medical-legal partnerships. It also identifies evidence gaps—particularly for older children and adolescents and for clinic-based food interventions—and summarizes implementation considerations such as screening uptake, navigation workflows, family-centered approaches, and building supportive, trusting relationships with parents and caregivers.
