On June 23 and 24, 2026, PHMI hosted a virtual learning session for participating health center teams, health center leaders, coaches, subject matter experts, and partner organizations.
Across two interactive half days, participants reflected on Action Period progress, shared peer examples, explored equity-centered improvement, designed small tests of change through an Instant Plan-Do-Study-Act (PDSA) session, and discussed strategies to strengthen population health management work in the next action period.
Session topics included quality improvement methods, data-driven implementation, health information technology, program and evaluation updates, financial sustainability amid policy shifts, peer problem-solving, and implementing clinical guidelines in routine care.
