The September Statewide Learning Session brought together community health center teams for two days of learning, collaboration, and planning focused on advancing Population Health Management (PHM). The event offered participants an opportunity to reflect on progress to date, learn from peer organizations, and prepare for the next phase of implementation and improvement.
