This session supported health center implementation teams in using data to identify opportunities for improvement, apply equity-centered approaches to population health management work, and consider practical next steps to improve care for all patients within their population of focus.

This session supported health center teams in identifying common PDSA challenges and designing a small, practical test of change to begin applying immediately in their population health management work.

Maximizing Technology for Population Health Management

This session explored how health information technology (HIT) can support goals, clinical outcomes, and sustainable improvement. The session covered how to assess opportunities to better use current tools, consider how HIT can strengthen plan-do-study-act (PDSA) cycles and implementation plans, and discussed how AI may support efficiency beyond the office visit.
