Business Case Executive Summary

The Business Case Guide helps organizations develop a financial model for an optimized care team to support population health management. It helps determine the expenses and potential revenues that result from investing in and then implementing population health management. This tool is currently designed for community health centers and how they are paid, but can be adapted by other practices.
