PHMI and Equity and Practice Transformation (EPT)

The Population Health Management Initiative (PHMI) is committed to improving quality of care and equitable health outcomes for Community Health Centers (CHCs) through initiatives such as Equity and Practice Transformation (EPT). EPT is a California Department of Health Care Services (DHCS) initiative to fund practice transformation models based off the collaboratively designed and evidence-based technical assistance approach created and implemented through PHMI for Medi-Cal primary care providers statewide.

The following CHCs from the PHMI cohort have been accepted into the EPT initiative:

Kaiser Permanente is proud to support the establishment of EPT’s new Population Health Learning Center, along with DHCS and the California Health Care Foundation. Building on the PHMI framework, the Population Health Learning Center will spread foundational resources and technical assistance approaches to support approximately that serve individuals enrolled in Medi-Cal across the state of California for the next 5 years of the EPT program. Thirteen out of 32 CHCs in the PHMI cohort will participate in the first round of EPT.

PHMI will continue to provide ongoing implementation support for its full cohort of 32 CHCs, including technical assistance, curriculum with resources to support population health improvement, grant funding for program and technology implementation support, and a customized PHMI Platform through the end of 2025. Based on its program evaluation, PHMI will explore scale and spread opportunities for other regions in California and nationally.
