PHMI has been working closely with a group of seven CHCs on Release 1 of the PHMI Platform – an opt-in technology solution that enables CHCs to strengthen their ability to provide equitable health care services, as well as drive improvements to produce more equitable health outcomes. Progress is underway, with a timeline to implement Release 1 of the Platform by the end of 2024. Key updates include:
- Release 1 Configuration
- Participating CHCs continue to provide input to ensure the PHMI Platform is tailored to meet key needs.
- Data Acquisition
- CHCs and the PHMI Platform Team are close to completing the interface between the CHC’s electronic health record and the PHMI Platform to enhance care before, during, and after patient visits.
- Third party data (e.g., claims, ADT data) are also being ingested into the PHMI Platform to enable CHCs to have a more holistic view of their patients, including insights into care gaps, care provided outside of the CHC, patients’ risk levels, and more.
- Health Information Technology (HIT) Assessment and Workflow Discovery
- PHMI HIT Advisors are supporting Release 1 CHCs to help determine how to effectively integrate the PHMI Platform into their existing workflows.
- Training
- Release 1 CHC “super users” have recently concluded a training series on the PHMI Platform that introduces them to the Platform and equips them to train their own colleagues at their respective CHCs.
- Site Visits
- Site visits to Release 1 CHCs are planned for the late summer / early fall to meet with leadership and provide updates on PHMI activities including PHMI Platform implementation progress.
Preparing for Release 2
As Release 1 implementation progresses, the team is also preparing for Release 2 of the PHMI Platform, with tentative go live set for late 2025. The PHMI Platform team is soliciting interest for a new cohort of CHCs for Release 2. As with Release 1, CHCs that participate in Release 2 will receive:
- Implementation support from HIT Advisors.
- Platform costs covered through 2028 and technology grant funding to support implementation.
- The benefit of lessons learned during Release 1 while helping to influence the Platform to best meet CHC needs.
PHMI Platform FAQ
For Release 1 CHCs, or any CHCs who may be interested in learning more about the PHMI Platform, a frequently asked questions (FAQ) document is now available on the Cohort Portal. The FAQ references various questions about the implementation and building of the Platform, including time commitment, funding, implementation alignment with program goals, EHR integration and specific product functionality details.
