Release 1 Implementation Activities
The PHMI Platform is an opt-in technology solution that enables CHCs to improve care quality and equitable health outcomes, and is meant to work in concert with other Health Information Technology solutions (e.g., EHR, Cozeva, etc.).
Go live preparations are underway for each CHC participating in Release 1 of the PHMI Platform. Prep for go live, which is scheduled to occur by the end of the year, includes completion of user acceptance testing and end user training.
The PHMI Platform has incorporated admissions, discharge, and transfer (ADT) data from the Los Angeles Network for Enhanced Services (LANES) and is in the process of ingesting claims data from MedPOINT. Combining these third-party data sources with the CHC’s own clinical data within the PHMI Platform provides a holistic view of the patient and relevant insights for pre-visit planning and at the point-of-care. In September, CHCs began testing the platform to validate data accuracy.
CHC Site Visits
The PHMI Platform team just wrapped up site visits with CHCs implementing Release 1 of the PHMI Platform. At each site, CHC leadership received a demo of the PHMI Platform in addition to a report out on implementation activities. The goal of the visit was for the PHMI Platform team to update CHC leadership on Release 1 and to hear from CHC leadership their questions, priorities, and goals for the PHMI Platform.
Donny Fan, Chief Information Officer, APLA Health, shared that the site visit was, “a very productive and encouraging collaboration among PHMI, Innovaccer, and our organization.”
Release 2 Preparation
Three demos conducted in August and September provided greater visibility into the PHMI Platform for CHCs that were interested in learning more.
The PHMI Platform team is planning to finalize the list of CHCs participating in Release 2 by the end of October.
