Recently, 6 CHCs participating in PHMI - East Valley Community Health Center, Via Care Community Health Center, APLA Health and Wellness, Northeast Valley Health Corporation, Mendocino Community Health Clinics, and Omni Family Health successfully adopted the PHMI Platform, an opt-in technology solution that enables CHCs to improve care quality and equitable health outcomes, and is meant to work in concert with other Health Information Technology solutions (e.g., EHR, Cozeva, etc.).
The Platform allows the health centers to better see care outside of the clinic with integration of clinical, claims, and admissions, discharge and transfer (ADT) data; as well as Uniform Data System (UDS) and HEDIS-based quality measures.
Further, these initial use cases address how to:
- Identify care delivery efforts outside of the clinic with integration of clinical, claims, and admissions, discharge and transfer data, which would reduce the need for care teams and providers to access multiple portals
- View quality measure performance through dashboards and action quality measures before and at the point of care, which would improve quality measure performance (and revenue through associated incentives)
- Enable targeted care team management between visits, which would improve care team efficiency and effectiveness

Working with CHCs to successfully implement the Platform marks a significant step in PHMI’s journey to leverage technological solutions to improve population health.

Additional CHCs are slated to go live with the Platform in the near future; it will continue to expand both its footprint and capabilities going forward.
