Person-Centered Workflows: Strategies for Development and Sustainability

In this interactive peer learning webinar, see how we tackle best practices and strategies for operationalizing clinical guidelines in person-centered, equitable and sustainable ways! We review two clinic workflows, including perinatal depression screening, as case studies to explore methods and tools for operationalizing clinical guidelines. The discussion also includes strategies for co-designing, testing and implementing clinic workflows, leveraging HIT and other clinic processes to hardwire changes, and tracking impacts of clinic workflows on key outcomes.

Right-sizing Panels: Today and Tomorrow

June 2024

This webinar explores how community health centers can right-size primary care panels, starting where they are today and how it could evolve for the future. Health center leaders, each at different stages of their empanelment journey, share insights about implementing an empanelment dashboard to optimize clinic operations. Participants review the purpose and methods of right-sizing patient panels using core metrics and assess readiness to implement panel weighting.

From Data to Action: Understanding Population Needs and Assets

May 2024

This webinar explores practical ways to learn more deeply about the needs and assets of your Population of Focus. Participants learn a tested method that enables teams to quickly generate concrete next steps in their population health management efforts. Multiple modalities and sources of learning are emphasized, including reviewing available data, engaging with people who care for the Population of Focus, and listening to patients.

LAN Peer Learning Kickoff

January 2024

This is the first LAN peer-learning opportunity. The PHMI Learning Action Network (LAN) supports our Community Health Center cohort with statewide and regional in-person and virtual peer learning opportunities. Webinars will introduce work around selected Populations of Focus, health information technology, equity, and social health. Combined, these efforts aim to educate and inspire your already stellar progress towards population health management.

Enhancing the Community Health Worker Workforce with CHCs

The Community Health Worker (CHW) workforce is accelerating as changes are creating new opportunities for community health centers (CHCs). This webinar reviewed the CHC approach on how to employ CHWs and operationalize CHW services at CHCs' sites and discussed the policies in place and take-home strategies to help the workforce grow sustainably.

PHMI Innovaccer Webinar
