Proof-of-Concept for the PHMI Platform Now Complete

For the past several months, the PHMI Technology Team has been conducting a proof-of-concept (POC) to validate the ability of a PHMI Platform candidate (powered by Innovaccer) to provide population health IT capabilities to meet PHMI Program and CHC needs. We are collaborating to improve the quality of care and address disparities for all patients and families served by California Community Health Centers.
